Help Members Take Charge of Their Health

When a person has asthma, congestive heart failure, or diabetes, even small lifestyle changes can mean big improvements in their health. Blue Cross of California Health Improvement Programs offer a fresh approach to helping members, together with their doctors, manage their condition through personal education and support.

The Health Coaching model

Health Improvement Programs are based on a Health Coaching model. A registered nurse Health Coach will work with members as part of a team of health professionals who might include dieticians, respiratory therapists, exercise physiologists, social workers, or health educators. Along with the personal physician, the Health Coach Team helps members feel confident and ready to take on their most challenging lifestyle changes successfully.

ASTHMA
ASTHMA
Poorly managed asthma can result in unnecessary visits to the hospital or emergency room. Blue Cross Health Coaches work closely with members to develop an effective routine that follows the physician's treatment plan and helps members avoid asthma "triggers."

DIABETES
DIABETES
Managing diabetes can help members avoid serious health complications including heart, kidney, nerve, and eye disease. While diabetes management is often overwhelming, Blue Cross dieticians, registered nurses, and exercise physiologists will help members work with their doctors to find ways to make positive and effective lifestyle changes.

CONGESTIVE HEART FAILURE (CHF)

Early identification of CHF symptoms can help avoid the need for emergency room visits and hospital admissions. Our CHF program is designed to help members recognize the early warning signs and better understand and work with their doctors to manage their condition.
